Abstract

To determine the prevalence oh HIV and other sexually transmitted infections (STI) in Escnintla. Guatemala, as well as identifying associated risk factors to aquirc STI among female sex workers (FSW), between June 2009 and February 2010.

We recruited 612 FSW with a median age of 26 years, attended in STI clinics located in public primary care centers in Escnintla. The participants were interviewed about sociodemographic characteristics and risk behaviors. Oropharingeal, gynaccologic and blood samples were taken to assess STI/HIV. To identify risk factors, to have at least one STI/HIV, we constructed a multivariable model and included all the significant variables associated with the outcome in the bivariable analysis and other clinically relevant variables.

The estimated HIV prevalence was 2.2%, syphilis 4.8%, Chlamydia 5.6%. trichomoniasis 9.5%. cervical gonococcal 0.9% and oropharyngeal gonococcal 0%. In multivariable analysis, a greater followup time in the six month screening visits STI clinics, as well as the practice of oral genital sex with steady partners suggested to be protective factors in presenting at least one STI (20.7% of FSW). The practice of vaginal douching and cervical alteration during physical examinations were considered risk factors.

HIV/ST1 prevalence among FSW was substantially high, except for gonococcus, which might reflect a low level of circulating bacteria as well as its susceptibility to the current antimicrobial treatment. In order to decrease the risk of infections, it is necessary to promote adherence to regular H1V/ST1 screening and include vaginal hygiene in prevention messages addressing FSW.
